Below you will find the answers to some of the most frequently asked questions about our services.

Yes and no, a physician referral is required by the State of Indiana for all MEDICARE patients to receive services from a physical therapist. All privately insured clients can directly access PT for 6 weeks before obtaining a physician referral- we will HELP with this.

Depending of the nature of your visit it will vary. An initial evaluation and treatment session will last between 45 minutes to 1 hour. Testing appointments will range from 1- 2 hours. Follow-up therapy appointments are usually 1-2 times a week for 3-8 weeks depending on the plan of care you and your therapist discuss.

Complete the patient portal and supplement forms as instructed by email you will receive from our front office staff- check your junk or spam folders. If no email, simply print your client forms located here and bring them completed to your scheduled appointment. Thoroughly complete your paperwork in its entirety. Bringing these completed forms to your initial visit will allow more quality time for you to spend with our therapists to aid in assisting you to a quick recovery. If you are unable to print and complete these forms, plan on arriving at least 15 minutes prior to your scheduled appointment to complete the required paperwork.

During your initial evaluation, a physical therapist will review the information you provide regarding your medical history and conduct a physical examination. This provides the means to accurately reach a diagnosis and develop a customized treatment plan. The physical therapist will communicate to you the details of the findings, make recommendations and work with you to design your individualized treatment program. You are encouraged to ask questions to help you comprehend anything involved in your plan of care. The first appointment usually ranges from 45 min to 1 hour in length.

After a thorough evaluation and diagnostic patient assessment, your physical therapist will create a comprehensive treatment plan tailored to your condition. A program of exercise and activities is designed for you to follow between visits to promote the ability to move, reduce dizziness, restore function and prevent disability or further injury. Clinical patient studies demonstrate an 85% resolution of symptoms when following our customized rehabilitation programs.

Be prepared to work! Most returning patients are expected to comply with their home exercise program as prescribed by their physical therapist. Patients are required to keep their scheduled weekly appointments and results must be achieved to continue the program. Patient’s typically return for 30 – 45 minute appointments weekly, depending on their individualized needs. You will work closely with your therapist one-on-one. We take this approach because we know the importance of individualized care for successful rehabilitation. When all treatment goals have been achieved, the physical therapist will create a comprehensive long term plan to assist you in maintaining your optimal health goals.

Depending on the type of testing you will be receiving, you will have specific instructions on how to prepare.

For hearing testing, please provide records of any prior hearing exams. Also make sure your ear canals are free from wax. All hearing tests are non-invasive.

For most types of vestibular testing we recommend all vertigo/dizzy medications, sleeping aids, and anti-histamines be stopped at least 24 hours prior to appointments. Refrain from caffeine and excessive sugar consumption prior to your visit. The length of the appointment usually ranges from 30 minutes to 1 ½ hours. VNG patients are encouraged to have a driver available as this test can make you dizzy.

Our licensed audiologist will provide a thorough evaluation using state-of-the-art diagnostic testing methods and equipment. Once the specific type and degree of your impairment has been determined we are able to provide comprehensive hearing services including a free hearing aid consultation and customized fitting of hearing aids. We can also help improve your quality of life by providing repair and cleaning of all makes and models of hearing aids and guidance in the selection of home assistive listening devices.

In-person and Telehealth appointments are available to both existing and new patients.

It is an insurance requirement that all co-pays, co-insurances, deductible and all out-of-pocket costs are due at the time of service. We will bill all appropriate insurances and submit claims forms on your behalf. As a courtesy to our patients we verify all primary insurance carriers to find out what your responsibility of payment is for each service.

Please note that our process of verifying coverage in no way guarantees payment for our services and it ultimately is your responsibility to know your insurance policy as it relates to coverage. You will be held liable for the payment of services. If for any reason you are not able to provide payment at the time of service, our billing office is available to assist you in developing a reasonable plan for payment.
